Jacob Michael Smith , 35, passed away Tuesday, September 1, 2015 at Clark Memorial Hospital in Jeffersonville, Indiana. He was born on May 15, 1980 to Bobbie and Teresa Smith in Fairfield, California. Jacob worked in the mental health field and was employed at Our Lady of Peace Hospital in Louisville, Kentucky. Jacob was preceded in death by his brother Jared Smith.

A funeral service will be held at 10:00 A.M. on Friday September 4, 2015 at the Garr Funeral Home in Sellersburg.
